Do you want some of your personal and clinical data shared automatically?

We have just been advised that NHS England have the power to automatically extract some of your personal and clinical data from our Computer system in order to improve future healthcare services for the NHS. This linked information will be used to create a new record, at this stage all identifiable data (such as NHS number, postcode and DOB) is removed and replaced with a code that does not reveal patient identity.

However, this extraction is beyond our control as a Practice – PLEASE TELL US if you DO NOT want your information automatically taken from our clinical system. If you wish to opt out of this scheme please complete the relevant form available from Reception, then hand your completed form back to Reception.
