The Community Nurses provide nursing care and advice to patients in their own homes. They liaise with hospitals and other specialised nurses and services such as Home Care, Social Services, The Macmillan Unit and Chiropody. They can arrange extra support for patients who require long-term care.
The Health Visiting Team are based in the surgery. Health Visitors are specialist nurses who work within the Primary Health Care team. They monitor child development, promote family and public health, offer advice on immunisation and run a weelky child health clinic.
Our Community Midwives work with the Doctors to provide care of mothers before and after delivery and care of babies in the first 28 days of life.
